1. No annual maximums.
2. Based on your own calendar year starting on the date the Plan is paid in full.
3. If the member does not utilize their savings Plan benefits, the Plan is non-refundable.
4. Payments for additional dental services are the member’s responsibility and are due on
the date of service to qualify for the discounts.
5. It is the member’s responsibility to schedule and keep all appointments offered as
part of the Plans. Please notify us at least 48 hours in advance if you must change
an appointment to avoid missed appointment fee.
6. These are not insurance plans.
7. These Plans may not be combined with any other offers, discounts, insurance, CareCredit, or advertisements.
8. Percentage discounts are off of usual, customary and reasonable (UCR) fees.
9. Whitening & bleaching products excluded.

Child plans age limit is 13 years of age.
If you have not had routine dental care, you may need a more extensive dental cleaning at an additional cost.
Bleaching/whitening products and implants are excluded from the discount program.
